HIPPA NOTICE OF PRIVACY PRACTICES

Jenison Public Schools (“Employer”) is committed to safeguarding the protected health insurance (“PHI”) of employees and their dependents enrolled in our group health benefits. Employer maintains a Participant Notice of Privacy Practices which summarizes our policies and your rights under HIPAA. A copy has been provided to you. You may request another copy at any time, free of charge, by contacting the Jenison Public Schools Administration office.

WOMEN'S HEALTH AND CANCER RIGHTS ACT

The Women’s Health and Cancer Rights Act of 1998, a federal law, provides certain rights to participants. Group health plan expenses for a mastectomy include charges for the reconstruction of the breast on which the mastectomy has been performed, surgery and reconstruction of the other breast to produce a symmetrical appearance, and prostheses and treatment of physical complications relating to all stages of the mastectomy, including lymphedema. Coverage will be provided in a manner determined in consultation with the attending physician and the patient.
